What insurance you actually need here.

Medical cover, surf injuries, volcano hikes and rental-car excess — what travel insurance for El Salvador should cover, and what the cheap policies leave out.

The short answer

You need medical cover with evacuation, and you need surfing and hiking explicitly listed. Private hospitals in San Salvador are good and expect payment up front. SafetyWing runs about $56 for four weeks; Heymondo costs more and covers adventure activities better.

Two things make insurance matter more here than the low crime rate suggests: the water and the roads.

What to make sure is covered

  • Medical, with evacuation. Serious trauma cases are often flown to Miami or Houston. That flight is a five-figure bill without cover.
  • Surfing. Reef cuts, dislocated shoulders and broken boards against rock are the routine injuries on this coast. Many policies exclude the sport by default.
  • Hiking above 2,000m. Santa Ana tops out at 2,381m, which some policies treat as mountaineering.
  • Rental car excess. Local agencies hold a deposit of $1,000–2,500. Excess cover is cheaper bought separately than at the counter.

The two I use

SafetyWing — around $56 per four weeks, subscription-style, good for long stays and digital nomads. Basic adventure cover, high excess, but the price is hard to argue with.

Heymondo — more expensive, better medical limits, and adventure sports included rather than bolted on. The right answer if the trip is a surf trip.

Before you buy

Check whether your credit card already provides medical cover abroad, and whether it covers the rental excess. Many premium cards do both, and then all you are buying is the gap.

Questions people actually ask

Is healthcare free in El Salvador?

Public healthcare is free at the point of use, including for foreigners in an emergency, but waits are long and the good facilities are private.

Does standard travel insurance cover surfing?

Often not. Most basic policies exclude surfing as an adventure sport or cover it only at extra cost. Read the activity schedule before you buy.

What does a hospital visit cost?

A private consultation runs $40–80. An overnight stay at Hospital de Diagnóstico in San Salvador runs several hundred dollars a day, payable or guaranteed on admission.
