Healthcare in El Salvador, for residents and visitors.

Public versus private healthcare in El Salvador — hospital quality, insurance costs, what a consultation runs, prescriptions, and when people fly out for treatment.

The short answer

Public healthcare is free at the point of use, including for foreigners in emergencies, but slow. Most residents use private hospitals — a consultation is $40–80 and private insurance for a healthy 40-year-old runs $60–120 a month. Complex surgery is often done abroad.

Healthcare here splits cleanly in three: the public system, the social-security system for formal employees, and private hospitals. Most foreign residents use the third and keep insurance for the rest.

Private care, and what it costs

  • GP consultation: $40–80
  • Specialist consultation: $60–120
  • Blood panel: $30–70
  • MRI: $300–500
  • Overnight private room: $200–400
  • Dental cleaning: $30–50

Those are cash prices, and cash is normal — many patients simply pay and claim back. Prescription medicines are inexpensive and most things that need a prescription elsewhere are available over the counter.

The hospitals

Hospital de Diagnóstico, San Salvador, is the reference private hospital: modern, English spoken in most departments, and where the diplomatic community goes. Hospital de la Mujer is the maternity standard.

Outside the capital, private clinics handle routine care well and serious cases get transferred. If you are settling on the coast or in the highlands, know in advance which hospital you would be driven to and how long it takes.

Insurance

Private cover for a healthy adult in their forties is $60–120 a month. Add evacuation cover — the reason is not the quality of routine care but the small set of things, mostly complex cardiac and oncology, that residents choose to have done in Miami or Houston.

Visitors should read the travel insurance guide; the calculus is different for a two-week trip.

The public system

Free, universal, and genuinely improved in the last decade, with new national hospitals built since 2021. It is also slow, crowded and short of specialists. In an emergency it will treat you and it will not ask about your immigration status. For everything else, most people who can afford the private route take it.
